A clerk with AMP Society in Sydney before enlisting in March 1941, FO McCullagh trained as a navigator under the Empire Air Training Scheme (EATS) and was posted to 460 Squadron RAAF at RAF Breighton, England. FO McCullagh was listed as missing when his Lancaster, serial number ED391, radio call-sign UV-S (S-Sugar) failed to return from an operational sortie over Berlin on the night of 29/30 March 1943.
