Shown are: 408882 Flight Sergeant (later Warrant Officer [WO]) Francis Edward Smallhorn, pilot, No. 2 Australian Air Ambulance Unit (1): 266409 Flight Lieutenant Ian Craig Morrison, Medical Officer, RAAF No. 5 Squadron (2): 135737 Leading Aircraftman Robert Howard Cook (4). WO Smallhorn was accidentally killed at Port Stephens, NSW while on a flight with 5 Operational Training Unit at Williamtown, NSW.
