| Notes | A review by Army Headquarters in 2013 has concluded that this original Recommendation for Honours or Awards for a Mention-in-Despatches (MID) for 2781632 PTE David Mark Buckwalter, and signed by LTCOL CM Townsend, was amended at Army Headquarters, without consulting the nominating officer or seeking his endorsement prior to processing the award. This action constituted a failure in due process resulting in maladministration and misrepresented PTE Buckwalter's involvement in the incident. As a result of these findings the Chief of Army, LTGEN DL Morrison, has reinstated this original Recommendation for the MID, as submitted by LTCOL Townsend, for the purposes of official records. All future references to the award should reflect the original citation as the recognised version. This decision in no way alters the level of the award or is intended to imply a different level of award than that which was received. |
